ABOUT US
We believe in treating the whole person using lifestyle medicine practices with an integration of family medicine, oral health, and nutrition. Very often, lifestyle interventions can be more effective than medications, with reduced side effects and risks. Recent studies published in the Journal of the American Medical Association describe the benefits of combined treatments of managed prescription medication with nutrition, exercise, and wellness therapies. Patients who receive lifestyle treatment plans for high cholesterol, obesity or weight gain, stress, diabetes, anxiety or depression may have reduced problems associated with their illness.
Our team will support you as you make progress to attain optimal health with the Coze Health® Care model using a team-based approach with physician, wellness coach, and nurse. We offer extended appointments that allow for our physicians to provide a treatment plan tailored to your health goals. Your Coze Health® Care will focus on reversing or living with chronic disease by incorporating emotional well-being, eating behaviors, activity, sleep disorders, and pain management into your care plan. We utilize a lifestyle monitoring app and patient portal to provide multiple ways to communicate your health steps and goals. Your physician will meet with your care team on a regular basis to review and update your treatment plan.
