We are excited to be able to offer you access to your healthcare records using the Elation Passport tool. But, did you know this is now mandated due to the 21st Century Cures Act that was passed in December of 2016? In order for us to be in compliance, we must provide you access to your healthcare record without charge and without delay. However, we will not provide printed health records.
How can you get access to your medical records? You must accept our invitation to the Elation Passport. It will be sent to your email that is registered at your first appointment. We can resend this invitation at any time you request. Once you receive the email, simply respond to the link and use a mobile phone to register. If you do not have a mobile phone, then you can schedule an appointment at the office and we will help you get your account set up. If you do not have an email address, you can schedule an appointment with a librarian at a public library who can help you get an email address (it is part of their mission!).
